Visiting Checklist

For first time patients at Ahn Medical, only after filling out our New Patient registration and consent forms will they be able to schedule an appointment with us. After an appointment has been made, all information and contact will be made through email and SMS messaging.

Below is a checklist for first time visitors/patients before their first appointment:

1. Valid ID: Real ID, Standard CA Drivers License, AB 60 Drivers License, Senior ID Card, Passport Book/Card, U.S Military ID, Permanent Resident Card (“Green Card”), etc.

2. Insurance Card

3. Medication List

4. Recent Blood Test or Imaging Results

5. Other CD/Images (if applicable)

6. Other Hospital Records
